"This should NEVER happen." So you believe that the probability of this event happening is zero. I hate to be the bearer of bad news but the probability of this event happening is definitely not zero.

I have been a member here for years and seen some crazy stuff. Nothing surprises me anymore and when I go into the play-offs, all bets are off. It is a crap shoot!

A major league season consists of what, about 2400 games? That's how many games PC simulates on any given day if memory serves. So 2400 games a day, 365 days a year, let's say 15 years. That's in excess of 13 million games of baseball. So if something is "one in a million" it should, statistically speaking, have happened 13 times. Hey, I won a championship. That's one of the 13 right there. :)

To be fair to OP, I don't think he meant that the situation literally should *never* happen. I think he meant that it happens more often in high leverage/playoff games than it should, in PC, because PC puts in the back of the bullpen in blowouts--without regard to whether it's regular season or a clinching game of the World Series. I know one potential improvement that's been discussed is a setting where the GM can specify whether to save the bullpen in a blowout or not, in a particular game.
