I just had a box score show the game being won by me after 10 innings even though all the pitchers' numbers total 12 innings. I scored one in the top of the tenth. It brought in my set-up and closer even though the game was tied and my settings say don't do that. Now my set-up and closer are OUT next game when they shouldn't be. On top of that, it bypassed more relief pitchers I had AND my long relief guy for a starting pitcher I had nowhere on any depth chart. I won anyway but it shouldn't have happened AT ALL!!! So how does this unbelievable malfunction happen at all?
It's all theoretical unless we search all your leagues for the phantom box score.

This game went 12 innings, no idea where you are getting that it was only 10. Your team scored in the 12th, not the 10th. The reason your setup and closer came in was because all your other RPs were tired from the last game, besides Sims who came in first. So at that point all you had left was SU and CL. After that, ALL your RPs were tired that’s why a bench rested SP came in. All that said, I think Unbelievable Malfunctions would be a great team name.

Game went 12 innings…nothing phantom or malfunctioning here; You are new to the site…so chalk this up to inexperience with the box score page

No and no. You see there, in English, where it says it bypassed my long reliever for a starter who was not on any depth chart. See that, in English, written out there, that you didn't read? You don't bypass who I say is the better pitcher and put in a guy who isn't listed on any depth chart. In English, that you didn't read, my long reliever wasn't used. And if this idiocy were to have gone an intelligent way, you'd bring in a long reliever BEFORE you bring in my closer IN A TIE GAME when I said NOT TO DO THAT and burn him for tomorrow in a game he might have saved. And if it was gong to bypass people, bypass my closer for this pitcher I didn't want in the game or I would have had him on a depth chart. Your actual defense of this is it's smarter to burn my closer in a tie game than burn either or both of TWO starting pitchers. Long relievers are rarely used in this "game" anyway but closers are called on OFTEN but burn my closer DESPITE MY SETTINGS. And you can't claim it put my closer in because he's the better pitcher if it put in a pitcher I say is WORSE than my long reliever or that on no depth chart pitcher WOULD HAVE BEEN ON A DEPTH CHART!
Long relievers have a setting where they will not enter the game after a certain inning (inning 3, 4, or 5 depending on what you set it at.) The set lineup page under bullpen clearly says in the box on the blue to the right that a long reliever is someone who "you want to enter the game when the starter gets sent to the showers early in the game (3rd inning and earlier)." That means the long reliever won't enter in extra innings.
This is a simulation site, there are some quirks (just like any simulation site), and it may take a little bit to get use to them all, but it is the best simulation site out there. You cannot control everything your team does (hence, simulation), but gbacci is very open to updating and making relevant changes when people are polite and civil with their suggestions. No reason to be rude right away. People here are willing to help if you are will to ask and discuss things nicely.
First off you had 3 RP - Minter, Loup, and Johnson who required Rest from the previous game. You starter got knocked out after 5 innings and the SIM started using the next best guys in order of what you had ranked, once it went through them it had only an SP who wasn't slotted and your Long Reliever who I have to assume you instructed the SIM NOT TO USE AFTER THE 5th Inning - so the Sim used the unslotted guy.
Nothing to see here, everything seems to have worked like it was supposed to, now if the unslotted guy had gotten used up too, THEN the SIM would have gone to your long guy.
I have no idea how you ended up with such lousy SP by the way, you used your 1st Supplemental Draft pick on Kenta Maeda - a 32 Ranked SP when Clayton Kershaw an 88 Ranked SP was still available???
You have some growing pains to go through, but we all did, well maybe not Gbacci....
